PAYMENT POLICY

Peak Physical Therapy & Wellness is an out-of-network outpatient physical therapy clinic. We empower patients with the flexibility and independence of seeking physical therapy with or without a doctor's referral. Accepted forms of payment include cash, check, credit/debit cards, or health savings account funds.

We do not submit claims directly with a patient's insurance company*, but we provide itemized receipts with the proper billing codes and information if a client chooses to self-file. We ask that clients pay at the time of service and the insurance company will reimburse the individual according to their specific plan.

If you are intending to file your own claims, we recommend contacting your specific insurance company first to ask what the benefits are for an out-of-network physical therapy provider and what paperwork is necessary to do so.

*MEDICARE BENEFICIARIES:  Peak Physical Therapy & Wellness is a non-participating provider with Medicare.  Federal regulations prohibit non-network physical therapists from collecting cash payments from Medicare beneficiaries for any problem that can be diagnosed. What that means is that we will personally submit claims for our clients who have Medicare, but patients will still need to pay for their visits at the time of service and Medicare will directly reimburse that individual.

However, if the service that is provided is not normally covered by insurance companies (wellness/education), then there will be no need to submit a claim.
